TKO YOUTH MENTOR APPLICATION
Please complete the following application to be considered as a mentor for the TKO Youth Mentorship Program.
Applicants must be between the ages of 14-18 and must not have a criminal background.
As part of your application, you must submit a one-page essay detailing why you want to become a TKO mentor and what makes you a great fit for the program. (Please note that you won't be able to save your progress and return later, so ensure you have your essay ready before starting the application.)
We also require that you include two personal references.
If you are under the age of 18 and accepted into the program, your parent or legal guardian will need to sign a consent form.
If you’re accepted into the program, will you agree to attend a one-hour in-person TKO training on bullying, ethics, and compliance with a member of TKO?
Were you referred to our program by someone at your school?
Do you have access to transportation, or someone to take you to your weekly mentoring sessions? (Please note that some mentees may meet on a bi-weekly schedule; once every two weeks.)
How many hours per week are you able to commit to the youth mentorship program? (Keep in mind that some activities may take longer than others.)
